Payment and Insurance Info

  • General Info

    Payment is due at the time of service, unless otherwise arranged. While you are required to enter your credit/debit card info at the time you book your appointment, you will not be charged at that time. Thus, you can choose a different form of payment when you come in for your session. Your card will only be charged if you do not show up for your appointment or cancel within a 24 hour period prior to your appointment time. (See 24 Hour Cancellation Policy on the Policies page.)

    PIP Patients will not be charged except in the event of not showing up for your appointment.

    Health Insurance patients will only be charged for their copay. However, in the event you do not show up for your appointment, you will be charged for the full cost of services.

  • Cash | Credit/Debit | Check | Venmo | HSA cards

    Payment is due at the time of service, unless otherwise arranged. Cash, credit cards, and HSA cards are welcome! While you are required to enter your credit/debit card info at the time you book your appointment, you will not be charged at that time. Thus, you can choose a different form of payment when you come in for your session. Your card will only be charged if you do not show up for your appointment or cancel within a 24 hour period prior to your appointment time. (See 24 Hour Cancellation Policy on Client Information page.)

  • Personal Injury Protection Claims

    PIP claims are accepted. Claim information will be collected via an intake form and will be used for billing purposes.

  • Health Insurance

    I am an in network preferred provider for most Kaiser Permanente health plans and Aetna PPO Plans. However, at this time, I am not accepting new Kaiser insurance patients. Beginning June 15th, I will be accepting new patients insured by an Aetna PPO Plan

    For insurance plans that have massage benefits for which I am not an in network/preferred provider, I can provide a superbill that you may submit to your insurance to request reimbursement. I am accepting out of network patients at this time.
